**Description:**
**_Position Purpose:_**
Our home care LPNs, Licensed Practical Nurse, use cutting edge technology and clinical knowledge to provide exceptional care to patients who require one-to-one attention and monitoring in their home. Home care LPNs are responsible for client assessment, care planning, intervention and overall supervision of patient care and outcomes. Our nurses follow physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
**_What You Will Do:_**
+ _Support Trinity Health at Home's mission, values, strategic goals and high standards of customer service._
+ _Adhere to the Corporate Compliance Program, including confidentiality of HIPAA protected health information._
+ _Responsible for ensuring outstanding customer service for all infusion services._
+ _Responsible for overall operation profitability. Monitors financial reports, recommends and implements necessary changes to meet and exceed budget expectations._
+ _Create an expectation of excellence and manages performance of direct reports to maximize the value of associates'' contributions towards organizational success._
**_Minimum Qualifications:_**
+ Graduate of an accredited or state approved school of practical nursing
+ Licensure as a LPN in the State of Ohio
+ **A minimum of One (1) years of experience in an acute care setting; home care experience preferred.**
+ Phlebotomy and wound care experience required.
+ Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
