LPN HH/HO - Skilled Nursing - Per Diem
Position Summary: Provides nursing care to patients under the supervision of a Registered Nurse. Education, License & Cert: High School Diploma or equivalent required. Graduate of an approved course in practical nursing. Licensed in the State of practice. Experience: One‐year experience in home care or public health nursing is desirable. Essential Functions: Acts as an associate nurse to deliver nursing care to patients based upon physician orders. Demonstrates effective time management, flexibility and priority setting in patient care. As an associate nurse, receives patient assignments and takes responsibility for ensuring the completion of patient revisits, participates in multidisciplinary case conferences. Demonstrates competence in performance of nursing skills. Practices within the scope of current policies and procedures of the nursing department and nursing guidelines. Regarding patient related documentation, the Home Care LPN is responsible for ensuring that he or she remains up to date in the most recent federal, state and agency requirements regarding content and time of submission. Collaborates with the RN, DOPS, patient and others in planning patient care. Demonstrates the ability to communicate observations of patients to fellow healthcare providers. Demonstrates respect and consideration for all patients as individuals. Maintains collaborative relationship with all members of the healthcare team. Other Duties: Travel for this position is always required. Patient Contact: Provides direct care to patients of all ages and interacts with family member when needed. Education, License & Certification: Working Conditions: Frequently exposed to disagreeable sights, sounds, odors, body waste, etc. Mental Demands: Routine duties requiring judgement in priorities and decisions with defined problems. Quality Statement: Caregivers must be active participants in quality improvement processes: 1. Identifying areas where quality improvement processes can be applied. 2. Collecting & organizing quality measurement data. 3. Participating on Quality Teams Job Hazards: Driving hazards on a daily basis. Some exposure to possible infections from patients with burns and open wounds. Degrees: Internal Contacts: Daily contact with clients at all levels, including managers and employees. Licenses and Certifications: Physical Demands: Occasionally required to push, move, and/or lift 100+ pounds. Considerable walking, standing, bending, reaching, pushing, pulling, lifting and sitting. Other Duties: Exposure Category: Tasks involve exposure to blood, body fluids, or other tissues, and communicable diseases. Equipment Used: External Contacts: Frequent contact with visitors, community agency reps and other healthcare related orgs.
