LPN - Community Health Worker

We are seeking an LPN with strong interpersonal skills to build relationships and interact effectively with individuals from diverse backgrounds and cultures. The ideal candidate will also have excellent organizational skills to manage multiple patients, maintain accurate records, and coordinate community health programs. Strong problem-solving abilities are essential to identify community health needs and develop appropriate intervention strategies. The ability to work independently as well as collaboratively within a multidisciplinary team is required. This position requires travel to various community settings, including homes, schools, and community centers, as well as performing office-based work. A valid driver's license and reliable transportation are necessary.   

Job Responsibilities:  

Providing medical care to members of specific communitiesAdvocating for health promotionEducating patients on disease preventionAdminister vaccines and medicationsEducate the community members about preventative care, nutrition, fitness, and hygieneAssess health needs and risks of individuals and populationsProvide accuracy and resource linkage for individuals and families in needDocument patient care servicesComply with public health legal requirements and professional standards 

Schedule: 

Full Time | Day Shift | 40 Hours Per Week | Monday - Friday | 8:00 a.m. - 4:30 p.m. | Flexible schedule with some weekends and evenings required

Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.

Requires graduation from a nationally accredited licensed practical nursing program.Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license.Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies.Successful completion of orientation required.1-2 years of relevant experience preferred.
