**Job Title:** Licensed Practical Nurse
**Department:** Infectious Disease
**Location:** Perelman Center for Advanced Medicine - 3400 Civic Center Blvd
**Hours:** Part Time - 20 hr/week

**Summary:**
The CPUP Licensed Practical Nurse assists physicians and performs healthcare services to patients in hospitals, clinics, and other healthcare facilities. Monitors and charts patients, collects sample, and performs other tasks as assigned, all under the direction of registered nurses and doctors.
**Responsibilities:**
+ Administers prescribed medications, gives injections and vaccines, reads PPD results.
+ Performs electrocardiograms (EKG) and screening tests.
+ Obtains and tests specimens following standard operating procedures and documents results, taking appropriate action with unexpected results.
+ Prepares specimens for transport as necessary.
+ Gather information from patient and direct to the RN or Provider when necessary.
+ Assists provider with surgical procedures including positioning patients, keeping a sterile field, and wound cleaning and dressing.
+ Prepares paperwork to complete patient visit.
+ Obtains and documents patient's vital signs as needed and reports abnormal results to provider.
+ Cleans, disinfects, sterilizes and checks to make sure all instruments and equipment are in proper working order.
+ Maintains medication storage closets.
+ Participates in orienting new team members.
+ Performs duties in accordance with Penn Medicine and entity values, policies, and procedures.
+ Identifies 2-3 participants per month meeting criteria for PMHARC Cohort and helping to recruit for other PMHARC studies.
+ Meet regularly (weekly or biweekly) with PMHARC staff members regarding recruitment for PMHARC Cohort and research studies and attend monthly PMHARC/CFAR Research Support Group.
+ Proficiency in harm reduction and patient centered care.
+ Ability to perform phlebotomy, wound care, and administer long-acting injectable medications in the medical office and during home visits.
+ Curiosity, humanity and communication skills for complex case management between multiple teams.
+ Work with clinic providers, social workers and staff to identify potential home visit candidates
+ Identify and refer patients to Penn LINK who would benefit from care navigation to facilitate linkage to MOUD care
+ Coordinate and track outreach/home visits including reaching out to patients prior to planned visit.
+ Other duties as assigned to support the unit, department, entity, and health system organization.
**Education, Credentials, and Experience:**
+ H.S. Diploma (Required)
+ Completion of Practical Nursing School (Required)
+ Practical Nursing License in the state of PA (Required)
+ BLS from the American Heart Association (Required)
+ 1 year outpatient experience preferred
