Job Summary

Dedicated to clinical support, our nurses provide professional nursing care while collaborating with providers, staff, patients and families. This role is a mix of both patient facing and non-patient facing responsibilities.
Responsibilities:
• Responds to and prioritize incoming telephone calls, and triage appropriately.
• Communicates with patients and family regarding medications, test results, and treatment instructions.
• Vaccine/Medication administration and Management -Vaccine inventory
• Population Health Management/Health Maintenance- Chronic Disease mgmt.
• Observes, records and reports patient’s condition and plan of care to the physicians/provider.
• Manages all aspects of process Prior Authorizations for: Rxs, DMEs, procedures, labs.
• In-Basket management.
• Ownership of Nurse Resource schedule for vaccines, BP checks, etc

Knowledge, Skills and Abilities
- Basic knowledge of patient education: interviewing and listening skills, teaching and engagement techniques, understanding of health-related conditions and information.
- Ability to accurately perform vital signs and obtain pertinent health information including but not limited to; temperature, pulse, respirations, blood pressure, height, and weight.
- Ability to administer medication; routes of administration may include injections, intravenous and oral medications.
- Knowledge and ability to perform specific procedures and tests as appropriate to the clinical setting.
- Ability to communicate in a professional, courteous, clear and concise manner.
