Description We are looking for a highly organized and proactive Logistics Coordinator to join our team on a Contract to permanent basis in Phoenix, Arizona. In this role, you will play a critical part in ensuring seamless patient placement and transfer processes within the healthcare setting. This position requires excellent communication skills, a customer-focused mindset, and the ability to work independently while collaborating with diverse teams.
Responsibilities:
• Coordinate the placement of patients by following physician orders and the guidance of the Patient Logistics Manager and RN.
• Manage patient transfers to appropriate facilities and units while ensuring accuracy and timeliness.
• Identify and escalate potential issues to supervisors or leadership to address and resolve barriers to efficient patient placement.
• Maintain clear and accurate documentation of all activities to support effective communication across care teams and facilities.
• Communicate effectively with staff from referring and receiving facilities to ensure smooth patient transitions.
• Foster collaborative relationships with colleagues, physicians, and care team members to facilitate timely and effective patient placement.
• Provide and accept constructive feedback in a thoughtful and precise manner to support continuous improvement.
• Demonstrate commitment to the organization’s mission and vision by maintaining a customer-oriented approach.
• Encourage innovative and creative solutions to enhance operational efficiency.
• Perform responsibilities independently with minimal supervision, ensuring high standards of quality and accountability. Requirements • Proven experience in patient logistics, scheduling, or a related healthcare role.
• Strong communication skills, with the ability to interact effectively with diverse teams and stakeholders.
• Demonstrated customer service expertise, with a focus on exceeding expectations.
• Familiarity with medical terminology and patient registration processes.
• Ability to document activities clearly and accurately for effective team communication.
• Problem-solving skills to identify and address barriers to patient placement.
• Self-motivated with the ability to work independently in a fast-paced environment.
