Position Summary
Under the supervision of the Program Director, LVN provides direct care and supervision of residents and observes and reports resident conditions to the Individual Service Plan Team.
Essential Functions
Provide compassionate and safe care for all residents. Respond to clients’ needs promptly, professionally, and with courtesy Provide care and supervision to residents as indicated in their Individual Service Plan including assistance with monitoring vital signs. Record/Report signs and symptoms of illness Recognize the need for assistance and or medical intervention, and act when necessary Monitor resident’s vitals, call the physician, call 911 or send to ER via taxi or ambulance as needed Follow all Alegria policies and procedures. Directly observe resident taking their medication, follow the medication policy and report procedure problems to the Registered Nurse. Complete end-of-shift report of residents and house activity to the oncoming shift and management staff. Always maintain resident confidentiality. Provide in-service education to staff and clients regarding medical care and universal precautions. Participate in and implement improvements through quality management process. Attend a monthly team meeting to review each resident’s progress. Update each resident’s Service Plan accordingly. Document all contact with residents and team members concerning individuals in the resident’s files. Maintain positive working relations with residents, staff, volunteers, Salvation Army representatives, and representatives from other agencies. Demonstrate self-direction and initiative in prioritizing, organizing and completing job responsibilities Maintain accurate patient records. Communicate effectively with the healthcare team. Educate patients and families on health maintenance, medication adherence, and disease prevention. Monitor patients for adverse reactions to medications. Participate in the training of new employees to the program. Participate in Individual Service Plan meetings. Participate in Program Meetings. Assist program Manager with evaluations. Other duties as assigned.Working Conditions
Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Ability to lift up to 50 lbs.
Minimum Qualifications
High School education or the equivalent. California Home Health Aid or Nursing Attendant Certification. Basic information on blood borne infection and universal precautions including the use of protective devices. Must pass criminal background check BCLS and Red Cross Certification Bilingual (English/Spanish) is preferred.Skills, Knowledge & Abilities
Must be culturally sensitive and non-judgmental Knowledge and sensitivity regarding issues of homelessness and/or HIV/AIDS Basic knowledge of the care needs of HIV individuals. Ability to work with diverse populations in a caring, sensitive, and non-judgmental manner. CPR/First Aid Certification
