Job Summary:

In accordance with the Purpose, Vision, Values and Coast Salish Teachings, and strategic direction of PHSA, safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

The Licensed Practical Nurse, Sunnyhill, in collaboration with a registered nurse (RN), provides care to patients with known health problems and predictable outcomes.  Duties include assessing, planning, implementing, and evaluating nursing care within a family centred, interdisciplinary clinical framework

Duties/Accountabilities:

1.Assesses and identifies patients’ and families’ actual potential strengths and limitations using physiological, psychological, social-cultural and spiritual needs by performing duties such as completing a nursing history and performing a head to toe assessment.

2.Contributes to the development and modification of individualized nursing care plans in     collaboration with the RN and input from the family by performing duties such as formulating actual and potential patient problem lists, identifying short and long term goals, providing input to the RN regarding prioritizing care, identifying nursing interventions, and evaluating and monitoring patient and family responses to nursing interventions.

3.Delivers and collaborates in planning and coordinating the nursing care of patients by documenting and updating information about the patient’s condition, reporting problems and/or potential problems to an RN and /or physician and changing the patient’s condition as required, and completing admission, teaching, discharge and transfer sheets.

4.Reviews and interprets the established nursing plan of care, provides nursing care and assists patients with hygiene, grooming, toileting needs, ambulation and mobility, and feeding, and performs procedures such as dressing changes and medication administration to assigned patients according to established policies and procedures.

5.Follows diagnostic treatment plans including collecting specimens and preparing and caring for patients undergoing diagnostic tests, examinations, or procedures.

6.Participates in and supports patient and family teaching to patient care and discharge planning.  Participates in the orientation of new LPN staff and LPN students, and assists in the orientation of other members of the health care team.

7.Performs other related duties as assigned.

Qualifications:

Education, Training and Experience

•Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Current full practicing licensure with the British Columbia College of Nurses & Midwives (BCCNM).

•Ability to operate related equipment.
