Licensed Practical Nurse - Outpatient - IVY II
Job Description Location: Ivy II - 602 Ivy Street, Elmira, NY 14905 + Department: Outpatient Primary Care What You’ll Do: As an Outpatient Licensed Practical Nurse (LPN), you’ll be responsible for providing patient care under the direction of a Registered Nurse (RN) or provider. You’ll work as part of a patient-centered care team to deliver coordinated care to a diverse group of patients, promoting optimal health and well-being. Your duties will include patient assessments, administering medications, performing various diagnostic and therapeutic procedures, and providing education to patients and families. Key Responsibilities: + Patient Care & Nursing Process + Provide direct care under RN or provider supervision, including collecting and analyzing patient data (physical, behavioral, cognitive). + Monitor changes in patient condition and communicate them effectively to the healthcare team. + Implement care plans and provide age-appropriate, evidence-based care. + Collaborate with care coordinators, case managers, and other team members to ensure patients receive comprehensive care. + Patient Education & Support + Educate patients and families on self-care techniques, health management, and disease prevention. + Provide information on medications, procedures, and follow-up care, ensuring clarity and understanding. + Assist in referrals to community services and disease management programs. + Documentation & Office Procedures + Accurately document patient interactions, care provided, and test results in the Electronic Medical Record (EMR). + Follow-up on lab specimens, test results, and provide accurate coding for billing purposes. + Assist with administrative tasks like scheduling, managing patient flow, and preparing billing documentation. + Team Collaboration & Office Responsibilities + Participate in care team meetings and quality assurance activities to ensure high standards of patient care. + Support other team members as needed, working within the scope of practice. + Maintain medical supplies and equipment, ensuring they are in good working order. + Safety & Emergency Preparedness + Follow safety protocols and demonstrate awareness of emergency procedures. + Maintain CPR certification and perform emergency tasks as necessary. + Professional Development + Participate in mandatory education and training sessions to stay up-to-date with best practices in patient care. + Continuously develop clinical skills and knowledge, actively participating in professional growth. What We Are Looking For: We are seeking a compassionate and skilled LPN who is dedicated to providing exceptional care to patients. You should be a team player with excellent communication skills, and a proactive approach to problem-solving. You must be able to work in a fast-paced outpatient setting, manage multiple responsibilities, and demonstrate attention to detail. Qualifications: + Education: Graduate of an approved LPN program with current licensure in New York. + Experience: Prior nursing experience in a medical office is preferred. + Certifications: CPR certification required within 90 days of hire. + Skills: Strong communication and documentation skills, proficiency in using EMR systems. + Physical Requirements: Ability to stand for long periods, lift, stoop, and reach as needed to assist in patient care and office procedures. Physical Demands: + Frequent standing and walking during patient care activities. + Some lifting, bending, reaching, and stooping. + Ability to work in a fast-paced clinical environment.
