Currently seeking Licensed Practical Nurses (LPNs) for days (6a-6p) in Polk City, Iowa. Candidates must hold a current Iowa LPN license in good standing. This role provides direct nursing care, supervises CNAs, and coordinates with a transdisciplinary team in a specialized long-term care setting for brain injury and neurological rehabilitation.
Position Highlights:
+ Schedule: Flexible
+ Hours: 6:00am-6:00pm
+ Location: Polk City, Iowa
Responsibilities of the Licensed Practical Nurse (LPN):
+ Deliver individualized nursing care in a long-term care rehabilitation setting.
+ Supervise and mentor Certified Nursing Assistants (CNAs).
+ Collaborate with transdisciplinary team members to support treatment goals.
+ Support activities of daily living and encourage independence.
+ Document and report changes in condition, and participate in care planning.
+ Help persons served achieve their highest quality of life through skilled nursing interventions.

Job Requirements of the Licensed Practical Nurse (LPN):
+ Current LPN license in good standing in the State of Iowa
+ Ability to maintain CPR certification (training provided if needed)
+ Must be available for rotating holiday shifts & every 3rd weekend shifts (unless weekend-only)
+ Must be available for on-call rotation as needed
