The Care Planning Partners program is a specialty geriatrics service providing geriatric and palliative care expertise and support within the primary care offices for patients with cognitive decline and/or patients who need advance care planning. The Ambulatory LPN will mirror the Provider schedule at the following locations: Stop 11 - Monday and Tuesday 5908 E. Stop 11 Rd. Indianapolis, IN 46237 Southpointe - Wednesday and Thursday 8920 Southpointe Dr., Ste B Indianapolis, IN 46227 County Line - Friday 333 E. County Line Rd., Ste B Greenwood, IN 46143 The Ambulatory LPN works hand-in-hand with the nurse practitioner to achieve quality outcomes for the patient. The Ambulatory LPN performs direct and indirect patient care under the supervision of the NP and collaborates on the plan of care. Direct patient care includes rooming patients and completing vital signs. Indirect patient care includes completing cognitive screenings, obtaining health histories, order transcription, documentation, and reinforcement of patient and family education. The Ambulatory LPN also collaborates with other team members to perform other unit services as directed. **Exceptional Skills and Qualifications** + Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience. (Required) + Previous experience in medical office or health care setting.
