Our Hospice Licensed Practical Nurses (LPN) are the heart of our organization! In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. 401(k) programEmployee Assistance Program (EAP)Vendor discounts Responsibilities Provide direct patient care to hospice patients, including appropriate pain/symptom managementImplement nursing practice following a comprehensive assessment and plan of careProvide accurate and timely documentation of patient servicesAssess and provide patient and family/caregiver educationParticipate in coordination of hospice servicesProvide appropriate pain/symptom management and evaluate patient response to treatments/medicationsParticipate in professional growth and development opportunities and quality assessment activitiesEnsure compliance with all state and federal legal and regulatory requirementsAdditional responsibilities as assigned Qualifications Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation

Three years nursing experience. Community health/hospice or medical/surgical experience is

preferred

Knowledge of the hospice philosophy of careCommitment to clinical and documentation excellence

Must be a licensed driver with an automobile that is insured in accordance with state and/or

organization requirements and is in good working order.

Possess and maintain current CPR certification.
