Job Description
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our Primary Care team. In this role, you will provide routine nursing care with a focus on high-risk and chronic care management support, as well as referral specialist support. Our organization is a Federally Qualified Health Center providing primary healthcare to children and adults in Greenwich and Stamford, with a special emphasis on populations living in or near public housing.
Responsibilities
Provide routine nursing care to patients.Focus on high-risk and chronic care management support.Coordinate care with elderly and disabled populations, particularly those with diabetes, HIV, hypertension, and obesity.Travel between Stamford and Greenwich as needed.Support referral specialist activities.Collaborate within a multidisciplinary team.Essential Skills
Current LPN licensure in CT.Minimum of 2 years of experience as an LPN.Experience in primary care or outpatient settings.Experience in high-risk or chronic care management.Knowledge of population health, care coordination, and referral processes.Proficiency with electronic health records (EHR).Excellent communication and interpersonal skills.Ability to work collaboratively within a multidisciplinary team.Bilingual English and Spanish preferred.Additional Skills & Qualifications
Strong organizational and time management skills.Compassionate, patient-centered approach to care.High attention to detail and ability to manage multiple responsibilities.Ability to work independently and as part of a team.Physical ability to stand and walk for up to 8 hours a day, stoop, kneel, crouch, and lift a minimum of 50 lbs.Excellent hand/eye coordination.Work Environment
Our Community Health Center operates Monday through Friday from 8 am to 5 pm, providing care to patients of all ages, from infants to geriatrics. We offer flexible scheduling within these hours and provide comprehensive training through onboarding, orientation, and shadowing, which can last between 2 to 4 weeks depending on the candidate's adaptability.

This is a Contract to hire position based out of Stamford, Connecticut.
Pay and Benefits
The pay range for this position is $33.65 - $36.06/hr.
