Licensed Practical Nurse
Position OverviewUnited Health Services (UHS) is seeking a Licensed Practical Nurse (LPN) to join our team and provide high-quality care to patients in a supportive and collaborative environment. As an LPN at UHS, you will work under the supervision of a Registered Nurse or physician to deliver compassionate, hands-on care, administer medications, monitor patient health, and assist with daily living activities.

$21.03 - $29.78 per hour, depending on experience

Job Responsibilities

Performs clinical assignments within scope of practice to maximize patient experience and ensure patient safety.Promotes and facilitates the compliance and understanding NYS DOH, Infection Control and all regulatory standards.Under the direction of a RN, educates patient and family in health promotion and disease management using available resources.Utilizes a working knowledge of pharmacology and safely administers medications according to UHS Home Care policies.Maintains CLIA Waived Lab standards and performs CLIA Waived Lab testing and proficiency validations according to regulatory standards.Collaborates with patients, families, and other members of the healthcare team to coordinate patient care.Participates in Performance Improvement activities, reviews PI data and assists in the development of Performance Improvement plans.Participates in educational opportunitiesWill perform other duties as assigned.

Position Qualifications:

Minimum Required:

Graduated an accredited school of Practical NursingCurrent New York State Practical Nurse License.Basic Skills Certifications (CPR) required upon hire.
