Hiring a Licensed Practical Nurse (LPN) in a homecare setting!

Job Description

As a Licensed Practical Nurse (LPN) in our home care division, you will play a crucial role in delivering comprehensive healthcare services to patients. You will work one-on-one in a patient's home, providing direct patient care, administering medications, and assisting with various healthcare procedures under the supervision of a registered nurse or physician. Your contributions will be vital in ensuring the well-being and recovery of those in need.

ResponsibilitiesConduct comprehensive assessments of patients' health status.Administer medications, treatments, and therapies as prescribed by the physician or registered nurse.Monitor patients' vital signs and report any significant changes or abnormalities to the appropriate healthcare provider.Assist patients with activities of daily living, including bathing, grooming, dressing, and toileting.Provide wound care, injections, and other skilled nursing services as needed.Educate patients and their families about health management techniques, medication administration, and disease prevention strategies.Maintain accurate and up-to-date patient records, including documentation of assessments, interventions, and outcomes.Collaborate with other members of the healthcare team to ensure continuity of care and optimal patient outcomes.Adhere to all regulatory requirements, policies, and procedures governing home healthcare services.Essential SkillsValid New York State licensure as a Licensed Practical Nurse (LPN).Current CPR certification.Strong clinical assessment and critical thinking skills.Excellent communication and interpersonal abilities.Ability to work independently and as part of a multidisciplinary team.Compassionate and patient-centered approach to care delivery.Reliable transportation and willingness to travel to patients' homes within the designated service area.Additional Skills & QualificationsExperience in home healthcare services is an advantage.Work Environment

Pay and Benefits

The pay range for this position is $72800.00 - $83200.00/yr.
