Description
Community Health Center of the North Country seeks a compassionate and dedicatedLicensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)to join our Federally Qualified Health Center’s Behavioral Health Department. Reports to:Medical Director / Practice Manager
Rate of Pay:$60,000 - $80,000 for full-time status; part-time or per diem shall be prorated based upon scheduled hours
Work Schedule:Flexible within Monday - Friday daytime office hours; no evenings, weekends, or major holidays; no on-call; full-time, part-time, or per diem schedules available

Position Summary:Responsible for providing individual therapy and/or counseling services to Health Center patients. This position also provides input and support for staff involved in the care of the patients.
Primary Responsibilities:
+ Perform psycho/social assessment.
+ Develop a therapeutic counseling regimen to meet the needs of the individual based upon assessment and input from caregivers/family.
+ Coordinate treatment plan with the psychiatrist, referring individuals as necessary for psychiatric consult and prescription.
+ Provide ongoing counseling as indicated and follow up if a psychiatric consultation is made.
+ Maintain patient records per agency policies and procedures.
+ Provide ongoing supervision of MSWs, as necessary and appropriate.
+ Perform other duties as requested by the supervisor.
Requirements
+ Master’s Degree in Social Work required.
+ Current, registered NYS LMSW license required.
+ At least 3 years of post-M.S.W. supervised experience in diagnosis, psychotherapy, and assessment-based treatment planning with a current, registered NYS LCSW license preferred
+ Experience with electronic medical records is preferred.
+ Strong organizational and planning skills.
+ Ability to maintain good working relationships with others.
+ Exhibits a high level of professionalism and confidentiality in dealing with issues.
+ Sedentary work; ability to occasionally lift and carry up to 25 lbs.
+ Ability to sit for long periods.
+ Ability to tolerate exposure to a computer monitor/screen.
+ Ability to tolerate the extended use of a computer keyboard.
