Job Description

Job Summary 

The social worker provides professional services to patients and families, which facilitates the use of individual, public and private resources so that optimal functioning can occur for both the patient and the family. These core services include assessment of psychosocial functioning and capacities, counseling for care planning and decision-making, social work treatment and community resource planning. Social workers collaborate with many disciplines, programs, departments and organizations in the accomplishment of these duties. 

Essential Functions

Provides assessment and counseling to psychiatric patients, their families and significant support systems. Assesses and provides discharge planning services to psychiatric patients within assigned unit. Conducts an initial psychosocial assessment interview with patients. Documents barriers to completion of assessment if applicable and places completed assessment within the medical record. Conducts both individual and group therapies for patient education related to but not limited to: disease process, emotional management, stress management, coping skills, decision making, and life skill development. Identifies and develops a plan of care related to patient’s emotional needs, community support system and on-going clinical concerns. Reviews and updates individual treatment plans as to patient progress, barriers, and continued discharge care plan needs. Performs other duties as assigned. Complies with all policies and standards.

Qualifications

Master's Degree in Social Work required 0-1 years Previous experience in individual, group, and family therapies required. required

Licenses and Certifications

LCSW- License Clinical Social Worker required
