Description:

Loyola Medicine, a member of Trinity Health, is a nationally ranked academic, quaternary care system based in Chicago's western suburbs. Loyola Medicine Transport is based at Loyola University Medical Center and is part of a three-hospital system including Loyola University Medical Center, Gottlieb Memorial Hospital, and MacNeal Hospital.  

The Licensed Clinical Social Worker, Transplant works as part of a multi-disciplinary transplant team, to provide individual counseling or referral for individual counseling as indicated by patient's assessment.

What you will do:

Works collaboratively with the behavioral health providers to include psychiatrists, psychologists and neuro psychologists for coordinated psychiatric patient care.

Provides biopsychosocial assessments, individual/family/group psychotherapy as needed as well as case management services as required.

Support the referrals from providers in the ambulatory setting seeking mental health, social and financial services for their patients.

What you will need:

Preferred two years acute hospital or transplant experience MSW from a CSWE accredited School of Social Work State of Illinois

LCSW; Licensed Clinical Social Worker State of Illinois

Compensation:

Salary Range: $29.07 - 46.51per hour 

Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
