JOB DESCRIPTION
Job Summary
Works collaboratively with Justice System agencies ensuring members receive care coordination and appropriate access to eligible and necessary services. Provides training and technical assistance to both internal and external stakeholders to enhance communication and coordination within the adult and juvenile systems of care. Acts as the single point of contact for information specific to members entering and exiting the justice system.
KNOWLEDGE/SKILLS/ABILITIES
Serves as the single point of contact to communicate and collaborate with the justice system (i.e., jails, Sheriff's departments, Correctional Health Services, and others)Acts as the interagency liaison with Department of Corrections (DOC, county jails, Sheriff's office, Correctional Health Services, Department of Juvenile Corrections (DJC), Office of Courts (OC) and probation departmentsEnsures appropriate transitions both in and out of the Justice System, including care coordination and completing documentation within the member's recordResponsible for Justice System reach-in initiatives, monthly or as needed reports and the development of a Justice Involved Strategic PlanInterfaces with the court coordinator regarding drug court, mental health court and criminal proceedings for membersShares information that assists the clinical team in developing treatment plans that incorporate community release conditionsCoordinates with providers, including delegated providers to ensure appropriate access to care and follow-upDevelops and initiates collaborative interagency protocols and MOU agreements within the court and justice systemsProvides coverage in the absence of the court coordinator, including attending and participating in court dispositions, coordinating court-ordered evaluation and treatment, and communicating follow-up needs with the internal care coordination staff.JOB QUALIFICATIONS
Required Education
Bachelor’s degree in Social Work, Nursing, Psychology, Criminal Justice or a related field, or equivalent experience
Required Experience
3 + years working within the criminal justice systemStrong verbal and written communication skillsDemonstrated problem solving skillsOrganized with strong attention to detailPreferred Education
Master’s degree in field of Psychology, Social Work, Criminal Justice Systems, or related field

Pay Range: $25.2 - $49.15 / HOURLY
