Possesses expert knowledge and clinical skill in order to oversee the day-to-day operations of the department, ensuring optimum quality patient/resident care outcomes.

Job Description

Establishes a rapport with patients/residents, families, and healthcare teams. Assists leadership with managerial activities such as: interviewing and hiring, employee schedules, counseling and educating employees, ensuring quality care issues are addressed and rounding with staff and patients/residents. Attends care conferences. Organizes and provides care plans for patients/residents. Enrolls and discharges patients/residents.

Demonstrates workflow management by assessing, planning, implementing, evaluating, supervising, and documenting the care of patients/residents. Delivers care in accordance with established standards of performance with patients/residents ranging in ages: infants, children, adolescents, adults and geriatrics. Possesses critical thinking skills for independent judgement as well as for active participation with the healthcare team.

Depending on location, may oversee minimum data set's (MID's), medical homecare programs, individualized education programming's (IEP's), medically related social service functions and lead medical home patient advisory meetings.

Applies knowledge and skills of generalist social work practice. Adheres to the National Association of Social Work Code of Ethics. Demonstrates knowledge of human behavior and developmental stages. Serves and/or chairs departmental/organization committees and task forces as needed.

Qualifications

Bachelor’s degree in Social Work from an accredited curriculum required.


Must possess a license in good standing in state(s) of practice:

In Minnesota:
Licensed Social Worker (LSW) or other allowed credential based on Minnesota statute

Hospital or healthcare experience preferred. Supervisor or leadership experience preferred. Minimum of two years’ experience in social agency and community organization dealing with health and/or welfare preferred.
