Description:Mount Carmel St. Ann’s is seeking a Lead Medication Historian to oversee and advance our Medication Historian Program. This leadership role is pivotal in ensuring accurate and comprehensive medication histories are obtained for our patients, enhancing safety and continuity of care.
As the Lead Medication Historian, you will supervise a dedicated team of medication historians, manage daily operations, and collaborate closely with the Clinical Pharmacy Manager to innovate and expand the program. Positions Purpose:

Certified Pharmacy Tech/ Medication Historian - Lead provides support for the interview process, evaluation process, establishing schedules, and adjusting/communicating assignments.

Quality and SafetyCalibration of compounding equipment as neededResponsible for complete and accurate required documentation in accordance with policy and regulatory entities·Environment - Maintains a clean, organized, and safe patient and work environmentMaintains an understanding of readying room temperature and pressure monitors for Environment Compliance in accordance with USP Chapters and Maintains knowledge of different medication classes and special handling requirements according to policies and standard operating proceduresCost effective utilization of supplies and resourcesMaintains working knowledge of standard operating procedures and standard work instructionsMaintains working knowledge of Pharmacy related USP Chapter UpdatesTechnologyUtilizes software systems appropriately and demonstrates continuous learningOversee technician training, development, including identification and correction of deficienciesReviews and assists with the establishment of operational policies and standard work instructionsAssists with monitoring regulatory compliance (quality assurance, safety, USP 797/800, USP 795, joint commission standards, Ohio State Board of Pharmacy rules etc)

Minimum Qualifications:

Education: High School or GED required· Licensure / Certification: Approved Pharmacy Technician Certification. Active registration as a Certified Tech with the Ohio State Board of Pharmacy· Experience: 2 year minimum experience, 3-5 years of experience in hospital pharmacy preferred.· Effective Communication SkillsRequires critical thinking skills, decisive judgement, and the ability to work with minimal supervision in accordance with Policies, Standard Operation Procedures, and State Laws· Must be able to work in a stressful and fast paced environmentMust be physically able to perform strenuous activity such as lifting, pushing, and repetitive movement.· Knowledge of relevant USP Chapters preferred
