servicesJob SummaryAs a Lead Community Recovery Coach this role is responsible for all CRC duties as well as some support duties to the program supervisor Involves work in community settings including clientsrsquo homes supportive housing andor residential programs as well as in local and state hospitals Assesses client needs plans and links appropriate services advocates for and monitors client needs to ensure the provision of comprehensive services to meet identified client needs Coordinates and provides services in an integrated Individualized Recovery Plan IRP Ongoing interaction with individuals who have been diagnosed with severe and chronic mental illness andor substance abuse diagnoses All employees in this position are considered ldquoEssential Employeesrdquo and may be required to provide mandatory overtime for program coverage needsJob ResponsibilitiesTransports clients to medical appointments pro social activities recovery activities and other necessary andor clinically appropriate appointments Also provides travel training to clients in the programUtilizes recovery oriented interventionsservices to promote recovery and independence and to achieve the goals identified on the IRPProvides supportive interventions teaching coaching mentoring and skills training to clients and their families in areas of recovery daily living skills community engagement and other areas that may be clinically appropriatePromotes and provides opportunities for engagement in natural support networks and activities relating to community integrationMaintains and assists in upkeep of program which may include but is not limited to supply orders grocery shopping basic cleaning duties inventory etcSupport scheduling management of Kronos PTO requests and management of call outsResponsible for supporting Supervisor with execution of weeklymonthly program schedules which may include foodcooking social activities community events house activities transition schedules and treatment team meetingsProvides supervised self administration of medications and complies with all Rushford policies and procedures Reports all actual and near miss errors immediatelyThrough collaboration with the client and other service providers assesses individual client needs and develops an Individualized Recovery Plan IRPCompletes and complies with all Rushford documentation policies and procedures as well as all Department of Mental Health and Addiction Services DMHAS and other funding sources documentation requirements This includes but is not limited to progress notes IRPrsquos case notes ROIrsquos admission and discharge paperwork and state reporting documentsHelps orient clients to the expectations of the program and enforces program expectationsOngoing and effective communication with Program Supervisor conservators key stakeholders family members and other supports and treatment providers involvedLeadscoaches teammates on leadership behaviors and job
