The Opportunity

Wages:$36.95-49.14
Reporting to the Site Manager or designate, you will complete social work assessments for new clients. You will also collaborate with other professionals to assess and identify clients' needs and participate in the development of care plans focusing on information identified in the social work assessment. You must record social work data and chart involvement with clients, assessments, interventions, progress, and discharge notes.
You must have a Bachelor of Social Work accompanied with current registration with the Alberta College of Social Workers or BC Association of Social Workers. Valid Commisioner of Oath registration and experience working in human services is required. Experience in a health care setting is preferred. Computer based technology as required by site. Experience with MDS is an asset.
