Intake Specialist (Weekend Choice)

Day Shift / 24hrs/wk / Weekend Choice

Schedule:  Every Thursday, Friday & Saturday

Responsible for a variety of administrative duties and support services for the Intake department including, but not limited to data entry,  faxing, filing, maintaining data bases, and monitoring department supplies. Processes referrals and gathers appropriate clinical documents for Home Health and Hospice admissions.

Essential Duties and Responsibilities:

Processes appropriate referrals and gathers clinical documentation per agency policy. Coordinates all Intake information in the management of referrals for home care and hospice. 

Assures coordination of all required services to clients and families by agency staff and community resources.

Assists with non-clinical support including data entry, faxing, scanning, filing, answering phones, calling referral sources, and ordering supplies

Maintains databases and pertinent information relative to the needs of the Intake Department.  

Works on special projects as needed.

Communicates effectively and tactfully with multiple referral sources, maintaining a customer service focus with outside agencies as well as branch personnel and colleagues.

Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements. Works weekends and holidays on a rotating basis as determined by department needs.

Participates in Weekend and Holiday rotation as needed and required.

Perform other duties as required.

MINIMUM QUALIFICATIONS:

High school Diploma or equivalencyExperience with computer data entry, word processing, and creation of reports.Maintains current drivers’ license and auto insurance.Possess reliable transportation (Exception: Employees who utilize public transportation—Supervisory approval required).
