Intake RN/Education Coordinator, Hospice
**Located in Cumberland, MD - UPMC Western Maryland is full of opportunities for those looking to take the next step in their career! We are currently hiring an Intake RN/Education Coordinator to support our Hospice team.** This is a full-time, dayshift position. May require weekend/holiday coverage as needed. The Intake RN/Education Coordinator receives and processes new referrals and contacts other agencies involved in the patient's care. Interfaces with physicians, hospital personnel, Care Management, and patient/client and family in the assessment, planning, and implementation for the patient's transition to hospice. Plans for and identifies immediate, short-term needs for the patient. Obtains needed authorizations from insurance companies. Coordination of education and competency training sessions for Hospice staff. Serves as a clinical resource for patients and family members. **Responsibilities:** + Possesses sufficient job skills and knowledge to perform the job in a competent manner. Is able to demonstrate skills and knowledge in day-to-day situations. + Communicates effectively and appropriately. Uses good judgment as to what to communicate to whom, as well as the best way to get that accomplished. Speaks in a clear and credible manner, selecting the right tone for the situation and audience. Listens to others and allows them to make their point. + Demonstrates that s/he is an effective team player who adds complementary skills and contributes valuable ideas, opinions, and feedback. Communicates in an open and candid manner and can be counted upon to fulfill any commitments made to others on the team. + Commits to continuous lifelong learning for self and others. + Associates in Nursing required. + BLS-Basic Life Support Certification is required, but will provide training if necessary. + 2-3 years experience in hospice or end of life care. + Good clinical and communication skills. **Licensure, Certifications, and Clearances:** + RN - Registered Nurse License - State of Maryland *Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state. **UPMC is an Equal Opportunity Employer/Disability/Veteran**
