Located in Cumberland, MD – UPMC Western Maryland is full of opportunities for those looking to take the next step in their career!  We are currently hiring an Intake RN/Education Coordinator to support our Hospice team. 

This is a full-time, dayshift position. May require weekend/holiday coverage as needed.

The Intake RN/Education Coordinator receives and processes new referrals and contacts other agencies involved in the patient’s care. Interfaces with physicians, hospital personnel, Care Management, and patient/client and family in the assessment, planning, and implementation for the patient’s transition to hospice. Plans for and identifies immediate, short-term needs for the patient. Obtains needed authorizations from insurance companies. Coordination of education and competency training sessions for Hospice staff. Serves as a clinical resource for patients and family members.
