Intake Patient Service Representative

Position Summary
Processes initial requests (intakes) for children and adolescents including clinical, demographic, and insurance information. Maintains intake and patient data . Gathers and scores all patient information and assessment tools, ensuring complete data is obtained prior toappointment. Refers intakes to Office supervisor for triage to appropriate physician
Primary Position Responsibilities
1. Conducts new patient intakes.
2. Maintains new patient intake log.
3. Scores all assessment tools.
4. Collects data.
5. Other duties as assigned
Qualifications
Minimum Education:

High School Diploma or Equivalent
Preferred:
Associate’s Degree
Minimum Years of Experience (Amount, Type and Variation):

Preferred:
1-2 years experience in a Special Education setting or physician practice preferred.
License, Registry or Certification:

Knowledge, Skills and/or Abilities:
Required:
Ability to work independently and interact with a variety of disciplines and systems. Knowledge of spreadsheets and basic computer literacy. Good communication, organizational, and strong customer service skills. Knowledge of pediatric disabilities and developmental milestones.
