Job Description
We are seeking an experienced Intake Nurse to support an OB clinic and Care Coordination team, working 32 hours per week. The role involves providing patient education, triage, and case management. Additionally, the nurse will manage patients' medical records and refer them to social services and programs when applicable. This position caters to a diverse patient population and collaborates closely with interpretive services.
Responsibilities
+ Conduct intake assessments for the OB clinic and the Chronic Disease clinic.
+ Support the Care Coordination team.
+ Assist the Telephonic Triage team located on site.

+ Operating appropriate patient care equipment
+ Accessing and entering computerized information

Additional Skills & Qualifications
+ Current Minnesota RN licensure
+ Current BLS/CPR certification
+ Prior experience in intake assessments strongly preferred
+ Spanish speaking is highly preferred
Work Environment
The position is on-site, working Monday to Friday from 8:30 AM to 5 PM, totaling 32 hours per week, with flexibility in scheduling.
Pay and Benefits
The pay range for this position is $38.00 - $45.00/hr.
