Job Title: Intake Nurse

Job Description

We are seeking an Intake Nurse for a prominent long-term care, skilled nursing, acute care, and hospice organization. This role involves coordinating referrals, verifying and entering physician's orders, identifying the level of care required by patients, reviewing documentation for facility placement, and coordinating orders with pharmacists. The position requires close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and families. Please note that this is an office-based position with no hands-on patient care involved.

ResponsibilitiesCoordinate referrals efficiently and accurately.Verify and enter physician's orders promptly.Identify the appropriate level of care required for patients.Review documentation to determine facility placement.Coordinate orders with pharmacists as needed.Collaborate closely with referring hospitals, physicians, skilled nursing facilities, patients, and families.Essential SkillsLicensed Practical Nurse (LPN) or Registered Nurse (RN) license in Utah.At least 1 year of experience, preferably in skilled nursing facilities.Experience in referral coordination and skilled nursing.Proficiency in long-term care processes.Additional Skills & QualificationsExperience with Matrix Care or Point of Care EMR.Business Casual dress code.BLS certification (nice to have)Work Environment

The role is office-based with a rotating Monday to Friday schedule between 8 am and 7 pm. You will work one weekend every 6-8 weeks, with adjusted days off accordingly, and be on call every 6-8 weeks. The team consists of 32 individuals in the office and 9 on the immediate team. The dress code is business casual.

Pay and Benefits

The pay range for this position is $32.00 - $32.00/hr.
