seamless care to patients Locate patients that transfer to hospital and communicate with clinical teams and TCCs Receive and resolve inquiries for referral dataitems required for coding and billing Effective communication skills self directed with a spirit of team support and success curiosity and ownership flexibility and a consistent demonstration of HW Leadership Behaviors and modeling Efficiency and accuracy in completing work as assigned Adherence to regulatory and agency policies and proceduresJob responsibilitiesCollaborate and communicate with Primary Care Providers and home care staff to ensure continuity of medical care to include obtaining clarifying validating service requests and completing verbal orders Communicate with transitional care staff clinical colleagues physicianrsquos offices and home care staff to coordinate homecare orders follow up appointments risk factors insurance parameters and goals of care Providing consultation to referral sources on community resources and home care issues Consistently communicate with HHCAH management to make sure all issues and problems are seamlessly handling so that both the patient and the referral sources are satisfied with the results and process Responding to internal and external communication timely and accuratelyEnsure collection and appropriateness of referral documents to support sound medical practice Reviews demographic and clinical pre admission documentation ensuring accuracy of information Reviews referring and transfer documents and medication list for accuracy and adherence to regulatory compliance and assuring the transitional care processes are implemented Adheres to the practice of confidentiality HIPAA and other statefederal regulations regarding patients families staff and the Agency Ensuring adherence with referral management protocols policies and proceduresBuilding and supporting positive effective relationships across the continuum and with the patients communities and the team Utilizing sound clinical judgment identifying risk and safety concerns and triaging appropriately Acts as a liaison to SSO HHCAH staff departments and customers both internally and externally Locate and follow up on transfer patients and communicates status to clinical teams and onsite TCCsAssist Homecare Customer Coordinator with FF requirements and MD verification when work volumes are high Demonstrates HW Leadership behaviors and supports and team in culture and team building initiatives Actively participates in the performance improvement process
