Intake Coordinator - Grace Medical Center - New Hope Treatment Center

Summary

JOB SUMMARY:

Under general clinical supervision, is responsible for the screening, admission assessment of patients, and recruitment of adults seeking substance use disorder treatment.  Conducts and supervises intake interview, registration procedure and insurance verifications.  Develops and maintains various logs, statistics and records pertinent to admissions and discharges.  Monitors implementation of patient treatment plans utilizing knowledge of chemical dependency and age-appropriate holistic growth and development issues to ensure that care provided meets the unique, physical and psychosocial needs of the patient.  Provides individual, group and family counseling services for a partial caseload.  Also provides addiction recovery education and case management, psychosocial referral, discharge and follow-up as core components of service.

Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.   Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures Receives all admission requests, screening calls and referrals. Conducts intake interviews with patients to establish their psychosocial and emotional status and to determine their suitability/readiness for the addiction services offered at one of the Substance Use Disorder Programs. Schedules prospective patients for intake and coordinates the process of enrollment, including case presentation at the weekly intake committee. Oversees the attainment of insurance information from the patient including verification of the carrier. Maintains patient census and tracks all demographic information to care system with emphasis on census, insurance, and referral source, etc.  Analyzes such data producing monthly reports. Working with Billing Clerk to negotiate length of stay factors with Carelon according to guidelines for insurability and evs’s patients. Maintains all inactive records and provides information to external agencies. Acts as liaison with agencies for referral and intake information. Manages patient caseload under general supervision and review. Member of Multidisciplinary Treatment Team. Collaborates with other team members to provide treatment that is tailored to the needs of each individual. Develops treatment plans for assigned patients by identifying problems and selecting methods of problem resolution. Interprets agency rules, regulations, policies, standards and objectives. Conducts individual, group and family counseling. Conducts interviews to gather information using standardized instruments: ASAM, Bio-Psychosocial Assessment, Addictions Severity Index (ASI) and MAST Test. Creates and monitors patient treatment contract within the levels of care and treatment phases protocols. Prepares monthly statistical caseload reports, completes discharge forms, Methasoft (if applicable) and keeps group attendance for assigned caseload. Conducts alcohol screening (breathalyzer readings) with the Alcosensor. Refers patients to appropriate resources for aftercare services. Develops and maintains effective relationships with other treatment center employees, physicians, volunteers and visitors to promote a positive workplace environment and to enhance the patient’s positive response to treatment. Maintains all patient records in network database (active and inactive); including urinalysis results, phases and levels of care, infractions, fees, medication regimes. Establishes and maintains patient files for accuracy and completeness of documentation in electronic health record. Maintains strict confidentiality of patient information as promulgated in federal law. Attends seminars, courses and lectures for CEU requirement to keep current with the field of Chemical Dependency treatment. Works closely with the Parole and Probation, Behavioral Health Systems Baltimore and other external agencies to maintain an effective referral relationship. Performs other duties as assigned.

REQUIREMENTS:

Bachelor’s degree in Health and Human Services Certified Addiction Counselor (CSC-AD) or higher level of Certification/Licensure in the State of Maryland Preferred Four (4) years of experience as a Chemical Dependency Counselor. Basic knowledge of addictions and its progression; familiarity with chemical dependency techniques and therapies; working knowledge of primary treatment Basic computer operation/literacy
