Inpatient Clinical Pharmacist - Aurora Medical Center Sheboygan County

Responsible for compounding and dispensing drugs to all customers and furnishing drug information to physicians and customers while in accordance with state and federal laws, and state-of-the-art pharmacy practice. Responsible for identification, resolution and prevention of drug related problems.

Reviews, evaluates, and consults with patients and other health care providers on issues relating to drug related problems and optimal drug therapy.

Checks to prevent allergic or adverse drug reactions and reports them through appropriate channels.

Reviews and interprets medication histories in order to make recommendations regarding drug therapy, or the process of drug delivery to other health care providers.

Reviews and evaluates drug related information to make recommendations regarding the most effective, cost-effective drug therapy. Reviews and accurately interprets physician orders

Clarifies the patient's understanding of their therapy to ensure the best possible outcome. Assures medication needs of patients are met throughout the continuum of care.

Prepares and distributes drugs in accordance with Wisconsin Administrative Code relating to the practice of Pharmacy, providing appropriate adjunctive information to the patient or intermediary.

Provides work direction and guidance for technicians in performing technical pharmacy functions. Is responsible for checking technician's work to maintain high standards of pharmaceutical care.

Provides input into establishing and implementing written policies and procedures for all pharmacy operations.

Assumes responsibility for operation of the department in the absence of the direct supervisor.

Licenses & Certifications

Pharmacist license issued by the state in which the team member practices,

must be able to be licensed in IL

Degrees

Bachelor's Degree in Pharmacy.

Strong customer service and organizational skills and confidentiality required. Excellent communication (written and verbal) and interpersonal skills. Demonstrated ability to work effectively with providers, staff and patients. Must be detail-oriented with the ability to handle multiple demands simultaneously. Intermediate computer skills including the ability to navigate through a database and run reports.
