Summary: As a part of the Goldbelt TAKU Team, you will be supporting the Naval Logistics Command with medical services for active-duty military personnel, their dependents, eligible DoD civilian employees, and other eligible beneficiaries designated by the Government. As a HealthCare Worker (HCW), you will be assigned to either a Military Treatment Facility (MTF) or other Federal medical treatment facilities in Japan. Responsibilities Essential Job Functions: Implement the MTF IPCP IAW AFI 44-108, Infection Prevention and Control Program and any guidance from the Defense Health Agency (DHA). Perform a semi-annual self-inspection to determine if there is sufficient evidence of MTF’s compliance or noncompliance with regulatory standards below. Provides assessment of IPC program to the Infection Prevention and Control Function (IPCF) Chair and shares with IPCF at quarterly meetings. Guides the MTF on IPC-related accreditation preparation activities and regulatory readiness. Formulates corrective actions IAW current standards and guidelines. Develops the annual Infection Control Risk Assessment (ICRA); prioritizes according to the level of probability and potential for harm. Coordinates ICRA development with other team members, including the IPCF Chair, IPCF members, Public Health (PH) and others as needed. Develops and coordinates the annual Infection Prevention and Control Plan (IPCP) based upon the approved ICRA prioritized list. Updates the IPCP as needed throughout the year. Develops and coordinates the IPCP Annual Summary that describes all activities in the IPCP. The summary shall include a general discussion, actions taken and resolution. Unresolved items must be carried over to the next IPCP Annual Summary. All program documents shall be accomplished in a timely manner; clear suspense for the completion of each document shall be signed by the Commander or designee and shall be in place thirty (30) days following the close out of the reporting year. Plans and develops the IPCF agenda with the IPCF Chair based on activities outlined in the IPCPP; standard agenda items are included at least quarterly. Completes and files meeting minutes per MTF policy. Contractor employees shall maintain active memberships in the following groups, or their equivalents: Safety or Environment of Care Committee, Product Evaluation Committee, Patient Safety Committee, Space Utilization Committee, Medical Readiness Staff Function, Nursing Executive Function, and Executive Committee of the Medical Staff (ECOMS). Performs IPC-related surveillance as defined by the annual ICRA and IPCP as approved by MTF senior leadership (e.g. ECOMS). Assists in reviewing and revising IPC relevant MTF Operating Instructions (OIs) as required. Shares surveillance activities and findings with the IPC Chair, IPC Function and other related stakeholders, as applicable. Verifies current standards are implemented throughout the MTF (e.g., reprocessing of reusable medical devices (RMD), high level disinfection (HLD), instrument processing, environmental hygiene, hand hygiene, transmission-based precautions, etc.). Participates in quality/performance improvement (PI) and patient safety (PS) activities related to IPC. Advises and collaborates on IPC-related issues with many various staff/groups that include, but are not limited to, IPCF, ECOMS, TJC Environment of Care program (EOC) Committee, Nurse Executive Council (NEC), and Performance Improvement Council. Liaises with installation Air Reserve Component (ARC) Medical Units on IPCP management and serves as a conduit to elevate IPC-related issues to MTF leadership. Assists ARC IPC monitor in development of a Memorandum of Understanding or Memorandum of Agreement (MOU/MOA), or Host Tenant Agreement. Consults with other MTF IPs, SG IP Consultant and DHA IP on a regular basis regarding data, trends, and activities. Collaborates with AFMS and local area peers to advance awareness of current IPC news and trends. Participates in AF and Defense Health Agency (DHA) IPC forums, teleconferences, and meetings. Applies epidemiologic principles and statistical methods to break the chain of infection and improve IPC efforts. Utilizes databases, graphs, and reports to track, trend, and monitor infectious processes and facilitate evidence-based practices. Uses Government-provided Information Technology (IT) systems; e.g., Electronic Healthcare Record (EHR), Composite Healthcare Systems (CHCS), etc., to identify infection occurrences, reservoirs, incubation periods, periods of communicability, modes of transmission, signs and symptoms, and susceptibility associated with the disease process. Interprets diagnostic and laboratory reports and advises clinicians on limitations and advantages of types of tests used to diagnose infectious processes. Identifies epidemiologically significant organisms for immediate review and investigation and collaborates with multidisciplinary teams, county/state public health departments, and when necessary, in consultation with the AF/SG IP Consultant, DHA IP and the CDC. Identifies indications for environmental and microbiological monitoring. Collects and compiles surveillance data from patient care areas and applies current nationally recognized definitions and methodologies to interpret and explain Healthcare Acquired Infection (HAI) data (e.g., Surgical Site Infections (SSIs), Standardized Infection Ratio (SIR), etc.). Collaborates with multidisciplinary teams on risk reduction and error prevention strategies to reduce HAIs. Interprets and translates data from antibiograms and antibiotic utilization. Educates staff on Multi-Drug Resistant Organisms (MDROs) and on optimal and prudent use of antimicrobials. Coordinates interprofessional effort to improve antimicrobial stewardship through education, surveillance, and evidence-based practices. Consults on facility renovations, construction and repair projects, facility modifications, and facility relocations impacting patient care areas. Develops a focused ICRA to preplan IPC efforts requiring coordination before any new construction or renovation begins. Conducts a walk-through of renovated areas prior to any unit occupying a new space or changes the existing functional use prior to the start of any construction project. Coordinates and consults on service contracts and plans that have IPC implications, to include the Hospital Aseptic Management System (HAMS) or equivalent housekeeping contract, linen/laundry contract, and waste management contract. Consults with Aerospace Medicine on development of exposure control plans. Collaborates with PS and Risk Management (RM) on tracking and trending of IPC near misses, events and/or issues relating to PS and the TJC National Patient Safety Goals (NPSGs) pertaining to IPC. Engages in proactive and reactive analyses in regards to IPC related events; e.g., Root Cause Analysis (RCA), Failure Mode and Effect Analysis (FMEA), or other PS initiatives. Advises leadership and staff on identifying relevant improvement opportunities and utilizes Performance Improvement (PI) tools; e.g., Plan, Do, Study, Act (PDSA), Lean Six Sigma, to improve performance and outcomes. Develops and implements MTF-wide IPC education and training. Facilitates training for professional and ancillary support staff on IPC topics. Assesses needs, develops goals and measurable objectives, and prepares lesson plans, power point slides, handouts and other materials for IPC educational in-services and briefings as needed. Prepares and provides briefings including, but not limited to, Newcomers’ Orientation, annual training, executive staff meetings, in-services, unit-based training and other programs as requested. Utilizes principles of adult learning to educational strategies and delivery of in-services and briefings. Prepares, presents, and coordinates educational workshops, lectures, discussion, simulation or one-on-oneinstruction on a variety of IPC topics. Evaluates the effectiveness of education and learner outcomes. Instructs patients, families, and visitors about methods to prevent and control infections. Maintains knowledge of IPC issues occurring world-wide as provided through valid communication sources (e.g. DHA, CDC, World Health Organization (WHO), APIC). Advocates, influences, motivates, and persuades leadership and staff to consistently incorporate IPC guidelines and standards throughout the MTF. Maintains IPC-related files IAW Air Force Manual (AFMAN) 33-363, Management of Records. Utilizes the APIC Text of Infection Control and Epidemiology on-line (http://text.apic.org/) and other IPC references located on the AFMS Virtual Library (i.e. https://kx2.afms.mil/kj/kx8/VirtualLibrary/Pages/home.aspx). APIC Text online access is granted by contacting the HQ IP (AFMOA). Include emergency preparation and management in the annual ICRA and IPCPP. Identify how an influx of infectious patients will be triaged and quarantined. Include emergency preparedness in Newcomers’ Orientation and Annual IPC Training. Identify required isolation and personal protective equipment (PPE) use. Participate in community-based and/or DoD exercises relevant to IPC disasters/emergencies. Qualifications Necessary Skills and Knowledge: Maintain a level of productivity comparable with that of other individuals performing similar services. Adhere to and comply with the Department of the Navy, Bureau of Medicine and Surgery, and local instructions and notices in effect during the term of the contract. Practice aseptic techniques as necessary; comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. Function with an awareness and application of safety procedures. Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner. Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF. Maintain an awareness of responsibility and accountability for own professional practice. Maintain DoD email account as directed by government supervisor. Exercise appropriate delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities. Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required. Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies. Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook. Minimum Qualifications: Minimum of a Bachelor’s degree in Nursing, Public Health, Epidemiology or Microbiology from a nationally recognized accredited college program. Prefer three (3) years of current experience in infection prevention and control performed within the past four (4) years. Must be a US citizen. Preferred Qualifications: A thorough understanding of the military lifestyle and experience working with military family members. Working knowledge of relevant military, state, federal, and local laws and resources. Highly developed written, oral, and presentation communication skills.
