In this role, you will assist clients in finding and maintaining supportive and affordable housing, working closely with the DBH Housing Unit and local resources. You’ll assess client needs, coordinate housing placements, and empower individuals with tools for long-term success. Your ability to build connections and advocate for services will play a key part in helping others thrive.

Key Responsibilities:
· Coordinate and support clients in their housing search and placement, advocating for affordable and supportive housing options
· Assess housing needs, strengths, and resources of clients and develop individualized support plans
· Provide coaching and training to promote housing stability and self-advocacy
· Collaborate with internal teams and community partners, including the DBH Housing Unit
· Refer clients to appropriate community services and advocate for access and quality
· Monitor and ensure coordination of services from external providers
· Participate in clinical team meetings to align on recovery plans and housing support
· Educate clients in accordance with rehabilitation and discharge plans
· Represent the organization at community events and committees as needed
· Adhere to organizational policies, procedures, and expectations
· Contribute to staff development activities and attend required meetings
· Provide services during weekends or evenings as needed

Education, Experience, and/or Credential Qualifications:
· Bachelor’s Degree from an accredited college or university in social work, psychology, nursing, or a closely related field
· Equivalent experience may be substituted on a one-year-for-one-year basis for educational training, with a minimum of a High School Diploma or GED

Additional Qualifications:
· Must be at least 18 years of age
· Valid driver’s license, acceptable driving record, and current auto insurance required
· Must successfully complete background check including criminal record, driving record, abuse/neglect, and fingerprint check
· Strong written and verbal communication skills
· Ability to follow directions, accept supervision, and establish effective relationships
· Must be able to apply common-sense understanding to solve problems and communicate effectively with residents, staff, families, and community partners

Physical Requirements
· Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects
· Repetitive movements of hands, fingers, and arms for typing or writing throughout the work shift
· Primarily sedentary work involving sitting, with occasional walking or standing
