Job Title: Housing Specialist

Job Summary:
As the Housing Specialist, you will assist. clients in accordance with an individual rehabilitation plan, in acquiring and maintaining supportive housing, room and board, and affordable housing. You will also actively work to pursue or advocate for expanding housing resources in the community and work closely with the DBH Housing Unit to access appropriate services.
Essential Job Functions:
Must have knowledge of the Clients Rights and ensure the client’s rights are not violated.
Will be expected to provide services at times requested by the Program Director or other members of the Recovery Services leadership team. This may include weekend and evening hours.
Participate in the clinical team meetings in order to identify strengths and needs related to development of the client’s recovery plan.
Assess clients for housing needs, strengths, and resources.
Arrange and refer for services and resources and when necessary advocate to obtain the services and quality of services to which the person is entitled.
Coordinates and facilitates housing search and placement, and coaches for success.
Have a working knowledge of community twelve-step recovery resources for the client and family members.
Monitor services by external providers to ensure communication and coordination of services.
Provide experiential training in using self-advocacy skills and resource acquisitions.
Provide information and education to an individual in accordance with the person’s rehabilitation plan and plan for discharge.
May be asked to participate in community events, media and committees as deemed necessary by leadership.
Functions in accordance with organization’s policies and procedures.
Participates in and contributes to the various staff development activities of the organization.
Participates in all required meetings.
Other duties as assigned by leadership.
Experience and Education Qualifications:
Bachelor’s Degree from an accredited college or university in social work, psychology, nursing or a closely related field.
Equivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED.
