Job Description:

Hospice RN Case Manager

Location: Macomb County
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Position Type: Full-Time | Field-Based

As a Hospice Registered Nurse Case Manager, you will play a central role in helping patients and families find peace and support at home during life’s most meaningful journey.

We are expanding our team in Macomb County and seeking a compassionate Registered Nurse who is ready to make a real difference.

--------------------------------------------------------------------------Key Responsibilities

Deliver skilled nursing care in patients’ homes with compassion and professionalism

Conduct initial and ongoing assessments, developing tailored care plans for each patient

Monitor changes in condition and adjust care as needed

Collaborate closely with physicians, social workers, and the interdisciplinary team

Educate families on disease progression, symptom management, and comfort care

Provide emotional support to patients and loved ones, helping them navigate end-of-life care

Ensure documentation accuracy and compliance with all care standards

Graduate of an accredited nursing program (Diploma, ASN, or BSN)

Current RN license in Michigan

At least 1 year of nursing experience (hospice or home health strongly preferred)

Reliable transportation and ability to travel throughout Macomb County

Strong clinical judgment, communication, and organizational skills

Physical ability to perform home visits, including lifting up to 50 lbs
