Hospice RN Case Manager

Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Service Area: Ludington to Manistee

Your Role & Impact

As a Hospice RN Case Manager, you are the heartbeat of our care team—delivering skilled nursing with a gentle, human touch. You’ll:

Create Individualized Plans based on physical, emotional, and environmental assessments.

Provide Hands-On Care that prioritizes comfort, symptom management, and quality of life.

Collaborate with Confidence by communicating vital updates to physicians, supervisors, and your interdisciplinary team.

Support Families Compassionately, guiding them through the hospice journey with education and empathy.

Act Proactively, quickly responding to any changes in patient condition and adjusting care plans as needed.

Deliver Excellence with every visit, upholding the highest standards of clinical care and patient dignity.

What You Bring

Graduation from an accredited nursing program (Diploma, ASN, or BSN)

Current RN license in Michigan

Minimum 1 year of clinical nursing experience

1 year of Hospice or Home Health experience is required

Reliable transportation and a commitment to patient-centered fieldwork

Ability to multitask, document thoroughly, and adapt with grace

Physical ability to lift up to 50 lbs and perform intermittent physical activity
