Full time Hospice Liaison RN -Albany
The Community Hospice has a rewarding opportunity for an RN to work as a part of our Hospice Team serving as Liaison in local Nursing Homes . HOURS : 8am-4:30pm (can be flexible). Position HighlightsAdvancement: Strong orientation program, generous tuition allowance and career developmentWork/Life: Positions and shifts to accommodate all schedulesNational mileage reimbursement is provided.What you will do:The Liaison RN evaluates patients for eligibility and interest in hospice services, describes the support and care available from hospice along with the benefits of hospice care, and coordinates with the patient’s community physician, facility, and our hospice medical directors to confirm eligibility for hospice services.Admits patients to hospice in the nursing home or ensures a smooth handoff to the hospice RN admitting the patient/family to services. The work schedule is flexible, generally M-F weekdays 8am to 4:30pm . What you will need:NYS RN LicensureASN required, BSN preferredRequires at least 1 year experience as an RN in an acute care settingHomecare experience requiredHospice experience preferredReliable vehicle and driver licenseExcellent communication skills. Must be able to communicate effectively with patients, physicians, and peers.Must be able to document key information concisely and accuratelyStrong computer skillsStrong organizational and time management skillsPay Range: $35.00-$47.75Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
