What You’ll DoServe as the primary RN case manager for hospice patients, ensuring care aligns with Elara Caring’s mission and vision
Assess patients and families to determine physical, emotional, psychosocial, and environmental needs
Document findings and care plans accurately in clinical records and interdisciplinary reports
Communicate significant changes or concerns to supervisors, physicians, and facility staff
Guide and educate patients, caregivers, and facility staff on hospice philosophy, pain control, and symptom management
Provide emotional and practical support during end-of-life transitions, including bereavement assessments
Collaborate with the Interdisciplinary Team to deliver the Right Care, at the Right Time, in the Right Place
What You BringGraduate of an accredited nursing program (Diploma, Associate, or Bachelor’s)
Current RN license in the state of practice
At least 1 year of clinical nursing experience (hospice or home health preferred)
Willingness to travel up to 50%
Physical ability to lift 50–100 lbs. and perform patient care tasks
Valid driver’s license, proof of insurance, and reliable transportation
