We’re seeking a PRN Hospice Registered Nurse—a professional with a true calling to provide comfort, dignity, and peace during one of life’s most meaningful journeys.

PRN/One to Two shifts per week from 10a-6p;

What You’ll Do as a Hospice RN

In this role, you’ll bring skill, compassion, and dedication to every patient and family you serve. Responsibilities include:

Promoting Elara Caring’s mission and ensuring quality, patient-centered care.

Maintaining patient and staff privacy in compliance with HIPAA.

Assessing patient needs (physical, psychosocial, and environmental) and creating individualized care plans.

Conducting initial evaluations and reassessments at each visit.

Communicating significant findings or changes to physicians, supervisors, and care team members.

Counseling patients and families on safety and care needs, using available community resources.

Implementing and revising care plans to support quality, continuity, and desired outcomes.

Providing skilled nursing services and following preventative/rehabilitative procedures.

Documenting care accurately, including medication regimens and updates.

Supervising, instructing, and supporting LPNs and Home Health Aides to ensure excellent care delivery.

Performing additional nursing duties as needed to support patients and the team.

What You’ll Need

Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing

Current RN license in state of practice

1+ year of clinical nursing experience (Home Health or Hospice preferred)

Current driver’s license, reliable transportation, and auto insurance (per state laws)

Ability and willingness to travel up to 50% within assigned branch territory
