This is afull-time, inpatient role focused on delivering exceptional, patient-centered care to individuals and families facing advanced illness.
Responsibilities
Key Responsibilities:• Provide comprehensive hospice and palliative care services in accordance with best clinical practices and patient goals of care.• Collaborate with physicians, nurses, social workers, and spiritual care providers to deliver interdisciplinary care.• Conduct patient assessments, develop care plans, and manage symptoms including pain, anxiety, and shortness of breath.• Communicate with patients and families about prognosis, treatment options, and end-of-life care planning.• Ensure accurate documentation and compliance with regulatory standards.

Qualifications
• Current Nurse Practitioner license in the state of Illinois.• Certification in Hospice and Palliative Nursing (preferred).• Strong communication, empathy, and decision-making skills.• Ability to work independently and as part of an interdisciplinary team.

Prime Healthcare Illinois Medical Group offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. Our Total Rewards package includes, but is not limited to, paid time off, a 401K retirement plan, medical, dental, and vision coverage, tuition reimbursement, and many more voluntary benefit options. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $120,000-$125,000 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
