Job Description:

Hospice Licensed Practical Nurse (LPN)

Elara Caring is looking for a passionate Licensed Practical Nurse to join our elite team of healthcare professionals and make a difference, one patient at a time. This position will cover patients in Livingston and Genesee County.

Schedule: Monday through Friday from 11:00 am to 7:00 pm

Coverage: Livingston/Genesee County

On an Average Day, you will:

Administering medications: Giving prescribed medicines to manage pain and symptoms.

Monitoring patient condition: Checking vital signs, observing symptoms, and reporting changes to the hospice nurse or doctor.

Providing personal care: Assisting with hygiene, bathing, dressing, and mobility as needed.

Comfort care: Helping to maintain comfort through repositioning, skin care, and other supportive measures.

Documentation: Keeping detailed records of patient care, symptoms, and any changes.

Emotional support: Offering compassionate support to patients and their families, answering questions, and sometimes helping with grief counseling.

Coordination: Working closely with hospice nurses, physicians, social workers, and chaplains to deliver holistic care.

What is Required?

Current LVN license as required by state

Strongly Preferred to live in the ​Livingston/Genesee County area

1 year of experience in an acute care or equivalent setting; preferred hospice or home health experience

CPR certification with American Heart Association or America Red Cross

Positive attitude

Dedication to quality patient care

Strong communication skills

Reliable transportation to perform job duties
