Hospice House Director
Hospice of San Joaquin  is the oldest not-for-profit hospice of 45 years.  We serve all of San Joaquin and Stanislaus Counties.  We are committed to continuing to be the leader in ensuring the quality of the journey, from a serious illness through end of life, and to honor our mission by honoring life by ensuring patients and their Caregivers are the focus of everything we do.  We are seeking a Dynamic, self-motivated Hospice House Director (HHD) who thrives in a fast-paced environment and supports our mission.  This is a Full-Time position reporting to the Chief Clinical Officer (CCO).   Position Purpose and Summary: Responsible for the management and coordination of the Hospice House. Ensure continuity of care, cost-effectiveness, and quality of operations within the facility. Facilitates communication and provides leadership to Hospice House staff, and other areas within the agency. Directly supervises Hospice House Staff.   Essential Duties and Responsibilities: - Responsible for coordinating and assuring high-level, quality care for hospice patients requiring inpatient, Respite, and Routine level of care within the Hospice House, as appropriate for the needs of each individual patient. Collaborates with each member of the interdisciplinary team to fulfill the Hospice of San Joaquin Mission. - Ensures the Hospice House is compliant with all Policies and Procedures, Hospice Medicare Regulations and Joint Commission Requirements, and state and federal law. - Provides a safe environment for patients, guests, visitors, and employees. - Adheres to policies and procedures and regulations related to patient care, conduct, ethics, safety, fire, security, and risk management. - Facilitates communication and works effectively between Hospice House staff, patients, families and caregivers, physicians, vendors, and other professionals as appropriate. - Serves as a liaison with other departments and vendors to resolve problems, and to improve efficiency and effectiveness as appropriate. - Reports problems or issues that cannot be resolved to the Chief Clinical Officer, or designee and makes recommendations for resolution if needed. - Functions as the agency’s Safety Officer and Emergency Management Supervisor. - Functions as the agency’s Infection Control Nurse. - Responsible for maintaining a Leadership role within Hospice of San Joaquin in order to fulfill the agency's mission. Consistently represents the Hospice House, both internally and externally, with integrity and professionalism. - Monitors documentation and implementation of physical, psychosocial, and spiritual interventions for the patient/family/ caregivers. - Responsible for the daily acuity of patients, and assists with patient care as needed for high acuity patients. - Performs evaluations of patients for admission and/or transfer for proper placement in the Hospice House at the correct level of care in accordance with CMS regulations. - Responsible for Hospice House Staff supervision and evaluation. - Responsible for the overall management of the Hospice House including but not limited to budget preparation, expense approvals, contracting issues, maintenance of supplies, and other management responsibilities as appropriate and designated by the CCO or CEO. - Ensures that patients/families/caregivers receive the education they need to understand the philosophy of hospice care, and the knowledge required to provide for the comfort and physical care needs of the patient. Provides education to the community as needed regarding the Hospice House and or the overall mission of the agency.   PRE‐EMPLOYMENT REQUIREMENTS: 1. Health Screening 2. TB Screening 3. Hepatitis B Series offered 4. Background Check 5. Reference Check 6. Drug Screening 7. DMV Screening   JOB QUALIFICATIONS: 1. A current license in the State of California as a Registered Nurse 2. One-year clinical experience dealing with acute care patients preferred. Six months of more of home care or Hospice experience preferred. 3. Bachelor’s degree or CHPN Certification preferred. 4. Basic knowledge of Microsoft Office products, including Word, Excel, PowerPoint 5. Ability to set priorities, and organize work independently. 6. Ability to make independent clinical decisions. 7. Knowledge of medications, indications, dosage ranges, side effects, and potential toxicity. 8. Skill in nursing practices with an understanding and knowledge of Hospice care and commitment to the philosophy and goals of the program 9. Ability to communicate effectively verbally and in writing. 10. Previous supervisory experience preferred.   PHYSICAL AND OTHER QUALIFICATIONS: 1. Ability and willingness to drive own vehicle, have a valid California Driver’s License and have automobile insurance that meets State standards. All employees, volunteers, consultants, and consultant volunteers who drive need to provide Hospice of San Joaquin evidence of insurance. This is necessary to be covered by the Agency’s non-owned auto policy. 2. Ability to safely maneuver equipment (wheelchairs, etc.) and assist family/caregivers with transferring and ambulating patients when necessary. 3. Implement personnel policies as they relate to nursing staff. 4. Physically able to walk, stand, stoop, sit, and lift. 5. Good manual dexterity. 6. Visual/hearing ability sufficient to comprehend written/verbal information. 7. Able to tolerate driving a car for long periods of time. 8. Able to negotiate steps and non‐paved surfaces to reach patients. 9. Able to move patients and/or bend over bed or wheelchair-bound patients while providing care.
