Schedule: Monday through Friday 8:00a to 5:00p (in office, not remote)

What You’ll Do as a Clinical Coordinator:Lead and Support Teams – Build, guide, and support care teams to ensure patients receive the highest quality of life and care.
Scheduling & Organization – Manage scheduling systems and communication processes that ensure seamless care delivery.
Collaborate Across Departments – Partner with clinical supervisors, physicians, caregivers, and staff to maintain patient-centered care.
Ensure Compliance & Quality – Monitor medical records, evaluate operations, and identify opportunities to improve workflows and productivity.
Promote Teamwork & Communication – Provide clear, supportive communication to staff, patients, and families while fostering a collaborative work culture.
Support Daily Operations – Assist with non-clinical inquiries, maintain departmental organization, and oversee documentation accuracy.

What We’re Looking For:Healthcare experience required – 1+ year in hospice, home health, or a related healthcare setting
Strong scheduling, organizational, and time-management skills
Proven ability to communicate effectively and work collaboratively with clinical teams
Knowledge of medical terminology, Medicare/Medicaid guidelines preferred
Proficiency with office technology and electronic documentation systems
High School Diploma; college degree preferred
