Hospice Clinical Coordinator

As a Clinical Coordinator, you’ll be the heartbeat of our care team, making sure patients receive seamless, personalized support while guiding and empowering the amazing clinicians who make it all possible.

Full-Time | Monday – Friday | 8:00 AM – 5:00 PM

What You’ll Do as a Clinical Coordinator

Lead and develop teams to provide the highest quality of life for our patients

Keep systems organized and efficient to ensure continuity of care

Monitor operations, track performance, and find ways to boost workflow and productivity

Support staff in maintaining accurate medical records and compliance

Serve as a key point of communication between the team, patients, and leadership

Take proactive steps to prevent avoidable hospitalizations and ER visits

Promote Elara Caring’s mission and values in every interaction

Protect patient and staff privacy in accordance with HIPAA

Jump in where needed to keep the department running smoothly

What You Bring

1+ years’ experience in hospice or home health care

Strong grasp of medical terminology

Experience working with clinical leadership teams

Proficiency with computers, fax, and copy machines

Familiarity with Medicare and Medicaid guidelines (preferred)
