You will: + Follow the established Plan of Care prior to each visit and deliver care under the direction of the RN and physician + Provide safe, compassionate care to patients, while educating and supporting caregivers when appropriate + Observe and report changes in condition, environment, or safety to the RN Case Manager or clinical supervisor + Promote patient safety and comfort while assisting with daily living tasks and mobility + Support patients in learning appropriate self-care techniques when possible + Offer emotional support to patients and families + Help reduce avoidable hospitalizations by observing warning signs and taking early action + Maintain accurate documentation and update medication records as directed + Communicate clearly and respectfully with families, care teams, physicians, and fellow staff + Treat every patient with dignity, compassion, and respect—always **Bring comfort.
