Remote Hospice Triage Registered Nurse (Part-Time)*Compact RN License RequiredAt Elara Caring, hospice care means helping patients live with dignity, comfort, and peace—wherever they call home. As a Hospice Triage Nurse, you’ll provide critical support to patients and families during evenings, nights, and weekends, ensuring their needs are met with compassion, expertise, and timely care decisions.
Schedule:
Week 1: Tues, Wed, Thurs, and Fri 4p-9p CST
Week 2 Sat & Sun 10a-10p + Mon 4p-9p
*this schedule would rotate

What you’ll need:Graduate of an accredited School of Nursing
Active Compact RN license in good standing (and be willing to get licensed in other non-compacted States)
3+ years of nursing experience, HIGHLY PREFERRED to be in hospice, palliative care, or Home Health
1+ year of leadership, triage, or after-hours clinical experience
Strong assessment skills and ability to prioritize care in high-stress situations
Compassionate communication skills with patients and families
Comfortable using technology and EMR systems remotely

Your Responsibilities:Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care.
Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
Plans, directs, and participates in delivery of professional services to hospice clients.
Communicates significant findings, problems, changes in condition or environment, and unsafe conditions to the physician and other personnel involved with patient care.
Initiates plan for patient safety, using the patient, family, and community resources.
Verifies the plan of care for the episode according to physician’s orders, assessment data, established standards and guidelines while identify and initiating actions based on deviations from normal nursing and condition.
Assumes primary responsibility for “hospice after hours supervisor” including assessing, planning, coordinating, implementing, and evaluating the plan of care for patients and families.
Maintains current knowledge of, and promotes compliance with all federal, state, and local regulations.
Keeps fully cognizant of current Medicare, Medicaid, and third-party reimbursement regulations.
Manages and directs field staff, weekend office staff, agency staff, other health care professionals, clients, and families related to appropriate and available health care resources to ensure the delivery of safe, effective, and appropriate hospice services.
Supervises, processes and coordinates patient referrals, physician orders, staff assignments, and patient re-certifications, and communications between various caregivers, the patient and their family, and the referring physician.
Completes death event documentation as required upon notification of patient death.
Determines appropriate need for spiritual or social work service and facilitates visits.
Identifies and instructs staff on admission criteria for new clients and evaluates eligibility for hospice care benefits.
Assures compliance with the requirements of state licensure, Medicare certification, and any other applicable oversight agencies through policy and procedure development, staff education, and ongoing monitoring activities.
Provides an environment which promotes respect for patient, privacy, and property.
Performs other duties/projects as assigned.
