Position Summary 

BridgingLife is seeking a Manager of Hospice Admissions and Referrals to lead and support our dedicated admissions team. This leadership role ensures optimal staffing, efficient patient intake processes, and strong relationships with referral sources and internal departments. The ideal candidate will be a proactive leader with a passion for quality hospice care and operational excellence. 

 

Key Responsibilities 

Ensure adequate and competent staffing to meet patient care and referral needs. 

Promote team satisfaction and retention through engagement strategies. 

Identify and address concerns in responsiveness, care delivery, and workflow. 

Develop interdepartmental relationships to improve patient and associate outcomes. 

Establish short- and long-term strategic goals based on metrics, audits, and industry changes. 

Coach and mentor staff using the Performance Management Process (PMP). 

Participate in departmental budget planning and ensure adherence to financial goals. 

Ensure availability of resources, supplies, and equipment for timely care delivery. 

Actively lead or participate in committees, LEAN initiatives, and community outreach. 

Foster associate involvement in departmental initiatives and strategic goals. 

Partner with the Executive Director to align team goals with organizational priorities. 

 

Qualifications 

Required: 

Active RN license with the Maryland Board of Nursing 

BLS certification through the American Heart Association 

Fleet Safety Required

3–5 years of nursing experience, preferably in hospice care 

Preferred: 

Certification in Hospice and Palliative Nursing (CHPN)
