Home Services Liaison
**Job Description:** We’re seeking a compassionate and organized Care Coordinator to serve as a vital link between hospital discharge and home health services. In this patient-centered role, you’ll ensure smooth transitions of care, provide exceptional customer service, and support patients in navigating their health journey from hospital to home.
