On Call: No
Home Infusion Clinical Liaison, RN Responsibilities include:
Responsible for facilitating transitions of care from acute and subacute settings to home infusion and education of home infusion and enteral therapies to hospital and community providers.
Assigned to selected facilities to collaborate with providers, case managers, referral sources, and home health agencies to facilitate patient discharge and coordination of care.
Responsible for collaborating with business development team in gaining and maintaining market share through referral intake process.
Responsible for timely communication with all referral sources telephonically, through electronic platforms, and in-person. Responsible for clear concise referral provided to pharmacy meeting all regulatory, payer, and safety requirements. Completes preadmission assessment and education to patient and caregiver of pharmacy services.
Conducts the initial infusion and enteral set-up and patient/caregiver training on our equipment and therapy services. Initiates formulary changes and recommendations on route of feeding and advances in nutrition via the direction of the pharmacy's registered dietitian.
Responsible for knowledge of Medicare and Managed Care regulations and requirements.
Reviews the patient's medical record to obtain both pertinent medical history and primary/secondary insurance information and communicates this to pharmacy intake.
Participates with any data collection required for therapy start and patient tracking processes, which includes facilitating the transfer of orders via phone, fax, and e-prescribing.
Timely response to referrals sources, providers, and leadership is essential. Monitors and communicates referral source activity acting as one point of contact for referral sources, home health, and infusion.
Will be responsible for additional transitions of care duties as assigned.
The Home Infusion Clinical Liaison, RN Requirements:
BLS (Basic Life Support)
Driver's License – State of Florida
RN (Registered Nurse) License
Required Associate's or Diploma Nursing
Required 2 years of Nursing; And – 1 year of Infusion Related
Required Business Development skills
Requires Reliable transportation
Requires infusion and enteral administration skills assessment
Preferred Computer proficiency in Microsoft Office and Excel
Preferred experience in Home Healthcare specialty Pharmacy
Preferred CRNI Certification
